Thank you for your participation in the FY2017 Shelby County Board of Commissioners Community Enhancement Grant Program which was reauthorized to provide assistance to certified 501 (c) (3) non-profit organizations and charitable organizations.
In accordance with resolution no. 12 approved on August 28, 2017, please complete the Community Enhancement Grant Program Year End Report. This form must be completed and should include a detailed accounting of how all grant funds expended were utilized by the award recipient. Additionally, please ensure that the Chief Financial Officer or Treasurer for the organization certifies the accuracy of the detailed itemized information being provided.
Please complete this report no later than September 30, 2017 or opportunities to receive future grant funds could be suspended.
